Most physicians finish training owing more than the price of a house, before they have even bought a house. After years of that number hanging overhead, a powerful instinct takes hold: kill all the debt, as fast as humanly possible. The feeling is completely understandable. It is also not always the move that leaves you wealthiest.

The reframe that helps is this: debt is not a moral failing to be punished. It is a mathematical tool to be evaluated. And two simple questions do most of the evaluating. What is the interest rate, and what is the debt actually buying?

At one end sits what most people would call bad debt. High interest rate, and it funded consumption that is already gone, the classic example being a credit card balance. At the other end sits what gets called good debt, or at least useful debt. A low interest rate, attached to something that holds or grows in value or produces income, a sensible mortgage being the usual example. Debt like this can quietly work in your favor, especially when inflation chips away at a fixed payment over the years while the asset behind it appreciates.

And then there is the gray area, which is where most physicians actually live. Moderate-rate student loans. The question of whether to throw extra dollars at the mortgage or invest them instead. The math piece is the concept of opportunity cost. The temperament piece is just as real: for some people, owning something free and clear delivers a kind of peace that no spreadsheet can capture.

The goal is simply to see your debts clearly enough to attack the truly costly ones, relax about the cheap and useful ones, and make a conscious call in the gray zone.
