Two to four hundred thousand dollars. For a lot of new physicians, that is the student loan number, and it can feel less like a balance and more like a sentence. But here is the shift in perspective that changes everything: it is not only a weight to be carried. It is a decision to be made, and the decision has real forks with real money riding on them.

The biggest fork, by far, is the choice between pursuing federal loan forgiveness and refinancing into a private loan. These two paths run in opposite directions, and the early choice is hard to undo, which is exactly why it deserves attention before you sign anything.

The forgiveness path, most commonly through the federal Public Service Loan Forgiveness program, rewards physicians who work for qualifying nonprofit or government employers. You make a set number of qualifying payments while employed by the right kind of organization, and the remaining balance is forgiven, tax-free, at the end. The catch is that it ties your loan strategy to the type of place you work, and the recertification paperwork is famously unforgiving of small mistakes.

The refinancing path runs the other way. You trade your federal loans for a private loan, usually to capture a lower interest rate. For a high-earning physician who is not chasing forgiveness, that lower rate can save a real amount of money over the life of the loan. The tradeoff is permanent, though: once you refinance federal loans into private, you give up federal protections and forgiveness eligibility for good.
